Physician / Practice Feedback Survey 

This questionnaire is intended for physicians and medical practices that treat injured workers. It should be completed by the physician or someone on the practice team who is familiar with the practice’s workers’ compensation processes.This survey is not an evaluation of any individual provider and is not used for marketing or sales purposes. All responses are used strictly for data analysis and program development.

About This Study
This survey is part of an ongoing, industry-wide study conducted by OM to better understand the administrative, operational, and system-related challenges practices experience when treating injured workers.The feedback collected from physician practices is used to identify common pain points, trends, and gaps in processes, communication, and workflow. This data helps OM continuously refine and improve its national standards, including the WorkCompReady™ Certification and the OM Center of Excellence.This survey is not a performance review and is not used for marketing or sales purposes. All responses are used strictly for data analysis and program improvement focused on practice operations and workers’ compensation processes.
